What is CIGNA Health Insurance?

Cigna is a large, nationwide insurance company covering nearly 18 million individuals in the US as of September 2025. They offer a wide range of coverage, including employer-insured plans, family plans, and government-sponsored programs. It is considered one of the leading health insurers in the US for physical and behavioral healthcare [1]. 

Cigna provides access to in-network treatment and approved out-of-network providers for services including detox, inpatient rehab, outpatient treatment programs, medication management, and individual and group therapy. 

CIGNA Benefits, Coverage, and Plans 

Copayments and deductibles vary significantly by plan types, levels, and treatment. Plan types help guide how you access care and manage referrals, while Cigna’s plan levels describe the cost shared between you and your insurer. Let’s look closer at some of these and what they offer [2]:

Cigna Plan Types 

  • HMO (Health Maintenance Organization): HMOs require you to choose a primary care doctor who can refer you to addiction treatment. These tend to be more affordable, with lower premiums and out-of-pocket costs. However, coverage is often limited to in-network providers, which reduces access to specialized services. 
  • PPO (Preferred provider organization): PPOs give you more freedom in choosing your provider, which can be in-network or out-of-network, and no referral is necessary from your physician. These have higher premiums and out-of-pocket costs but are ideal with individuals who need access to a wide range of specialized mental health and substance abuse treatment services. 
  • EPO (Exclusive Provider Organization): Similar to HMOs, EPOs offer lower deductibles and some savings, though not as significant as those of HMOs. They cover a wider range of providers, and no referrals are needed for in-network providers. 
  • POS (Points of Service): These combine elements of HMOs and PPOs. You can receive out-of-network care, but at an increased cost. They typically do require a referral for specialized behavioral health services. 

Cigna also works with Medicare and Medicaid through specialized programs to expand access to care for eligible individuals. For Medicare, Cigna offers Medicare Advantage (Part C) plans, Medicare Part D prescription drug plans, and Medicare Supplement plans in many states [3].

Cigna Plan Levels

  • Bronze: Lowest monthly premiums but higher deductibles and out-of-pocket costs, covering about 60% of healthcare expenses.
  • Silver: Moderate premiums and cost-sharing, paying roughly 70% of expenses.
  • Gold: Higher premiums but lower out-of-pocket costs, covering about 80% of expenses.
  • Platinum: The most comprehensive and expensive plan level, covering about 90% of healthcare costs with minimal out-of-pocket spending.

What Level of Mental Health and Addiction Treatment Does Cigna Cover? 

Cigna covers several, if not all, levels of care for drug and alcohol treatment, including detox, inpatient rehab, partial hospitalization programs, and aftercare/outpatient therapy programs. This is mandated by the Affordable Care Act (ACA), which requires providers to cover at least a portion of addiction treatment [4]. 

Cigna also covers a range of mental health conditions, evidence-based therapies, and psychiatric services for prescription medication, as mandated by the ACA. It covers most, if not all, mental health conditions, including depression, anxiety, trauma disorders, bipolar disorder, and ADHD. Cigna also supports telehealth options, making care more accessible

Coverage for treatment depends widely on your plan, level of need, and whether the facility, therapist, or treatment provider is in-network. Typical mental health therapies such as CBT, DBT, and EMDR are typically covered by CIGNA insurance.
