Verifying insurance for someone else

Anyone can check insurance for rehab for the man who’s going. Plenty of these forms come from a wife, a parent, or an employer’s EAP. Start it for him, and admissions takes it from there.

Still have a question? (928) 377-5230

The insurance check is free, and every decision after it is yours. Admissions calls your insurer, checks your benefits for residential treatment and calls you back with what they found. Then you choose whether AnchorPoint is the right next step, in your own time.

Usually the same day. Admissions is open 24 hours, so you can send the form at night or on the weekend and a real person will call. Once you have your answers, you decide what’s next.

Yes. A wife or parent often fills it out for him, and employers and EAPs can start it with his OK. Use his insurance details and pick “Someone I love” or “A client or patient” on the form, and admissions takes it from there.

Yes. We check both in-network and out-of-network benefits. Out-of-network means the program works outside a contract with your plan, and your plan may still help pay; many PPO plans do.

Just your name and a phone number. Your insurance plan, member ID and the policyholder’s date of birth help us check faster, so have your card handy if you can. Admissions can also go through the details with you on the call.

AnchorPoint works with private insurance, including PPO, employer and union plans, plus TRICARE and TriWest. For Medicare or Medicaid (AHCCCS), Admissions can help you find a program that takes them. If you have private insurance, send the form and we’ll check it for free.

AnchorPoint is in network with UnitedHealthcare, and we work with Aetna, Cigna, Blue Cross Blue Shield, TRICARE and TriWest. Most PPO plans help pay out-of-network. For any other plan, choose “Another plan” on the form and we’ll check it. What any plan pays depends on your own benefits.
